Health Care Systems – Four Basic Models

An excerpt from correspondent T.R. Reid s upcoming book on international health care, titled We re Number 37!, referring to the U.S. s ranking in the World Health Organization 2000 World Health Report. The book is scheduled to be published by Penguin Press in early 2009.

There are about 200 countries on our planet, and each country devises its own set of arrangements for meeting the three basic goals of a health care system: keeping people healthy, treating the sick, and protecting families against financial ruin from medical bills.

But we don t have to study 200 different systems to get a picture of how other countries manage health care. For all the local variations, health care systems tend to follow general patterns. There are four basic systems:

The Beveridge Model

Named after William Beveridge, the daring social reformer who designed Britain s National Health Service. In this system, health care is provided and financed by the government through tax payments, just like the police force or the public library.

Many, but not all, hospitals and clinics are owned by the government; some doctors are government employees, but there are also private doctors who collect their fees from the government. In Britain, you never get a doctor bill. These systems tend to have low costs per capita, because the government, as the sole payer, controls what doctors can do and what they can charge.

Countries using the Beveridge plan or variations on it include its birthplace Great Britain, Spain, most of Scandinavia and New Zealand. Hong Kong still has its own Beveridge-style health care, because the populace simply refused to give it up when the Chinese took over that former British colony in 1997. Cuba represents the extreme application of the Beveridge approach; it is probably the world s purest example of total government control.

The Bismarck Model

Named for the Prussian Chancellor Otto von Bismarck, who invented the welfare state as part of the unification of Germany in the 19th century. Despite its European heritage, this system of providing health care would look fairly familiar to Americans. It uses an insurance system the insurers are called sickness funds usually financed jointly by employers and employees through payroll deduction.

Unlike the U.S. insurance industry, though, Bismarck-type health insurance plans have to cover everybody, and they don t make a profit. Doctors and hospitals tend to be private in Bismarck countries; Japan has more private hospitals than the U.S. Although this is a multi-payer model Germany has about 240 different funds tight regulation gives government much of the cost-control clout that the single-payer Beveridge Model provides.

The Bismarck model is found in Germany, of course, and France, Belgium, the Netherlands, Japan, Switzerland, and, to a degree, in Latin America.

The National Health Insurance Model

This system has elements of both Beveridge and Bismarck. It uses private-sector providers, but payment comes from a government-run insurance program that every citizen pays into. Since there s no need for marketing, no financial motive to deny claims and no profit, these universal insurance programs tend to be cheaper and much simpler administratively than American-style for-profit insurance.

The single payer tends to have considerable market power to negotiate for lower prices; Canada s system, for example, has negotiated such low prices from pharmaceutical companies that Americans have spurned their own drug stores to buy pills north of the border. National Health Insurance plans also control costs by limiting the medical services they will pay for, or by making patients wait to be treated.

The classic NHI system is found in Canada, but some newly industrialized countries Taiwan and South Korea, for example have also adopted the NHI model.

The Out-of-Pocket Model

Only the developed, industrialized countries perhaps 40 of the world s 200 countries have established health care systems. Most of the nations on the planet are too poor and too disorganized to provide any kind of mass medical care. The basic rule in such countries is that the rich get medical care; the poor stay sick or die.

In rural regions of Africa, India, China and South America, hundreds of millions of people go their whole lives without ever seeing a doctor. They may have access, though, to a village healer using home-brewed remedies that may or not be effective against disease.

In the poor world, patients can sometimes scratch together enough money to pay a doctor bill; otherwise, they pay in potatoes or goat s milk or child care or whatever else they may have to give. If they have nothing, they don t get medical care.

These four models should be fairly easy for Americans to understand because we have elements of all of them in our fragmented national health care apparatus. When it comes to treating veterans, we re Britain or Cuba. For Americans over the age of 65 on Medicare, we re Canada. For working Americans who get insurance on the job, we re Germany.

For the 15 percent of the population who have no health insurance, the United States is Cambodia or Burkina Faso or rural India, with access to a doctor available if you can pay the bill out-of-pocket at the time of treatment or if you re sick enough to be admitted to the emergency ward at the public hospital.

The United States is unlike every other country because it maintains so many separate systems for separate classes of people. All the other countries have settled on one model for everybody. This is much simpler than the U.S. system; it s fairer and cheaper, too.

Note – Reid s Beveridge model corresponds to what PNHP would call a single payer national health service (UK); Bismark model refers to countries that PNHP would say use non-profit sickness funds or a social insurance model (Germany); and National health insurance corresponds to single payer national health insurance (Canada, Taiwan). Reid s out-of-pocket model is what PNHP would call market driven health care. Some countries have mixed models (e.g. Sweden has some features of a national health service such as hospitals run by county government; but other features of national health insurance such as physicians being paid on a FFS basis). This explains why Reid might classify the Scandinavian systems as Beveridge while PNHP classifies them as single payer national health insurance.

Do Global Business students have to study abroad?

I’m not a Global Business student. Can I still study abroad?

Yes – they need to either have one semester-long exchange program, one international internship, or two JGE courses.

Can students complete their study abroad and their internship requirement at the same time?

Yes! International internships are encouraged!

What semester is the best to study abroad?

  • The ideal time to study abroad is Sophomore/Junior year. NO student (Undergraduate or Graduate) can participate in an exchange program or Third Party Program during their last two semesters at UTD.
  • Students can participate in JGE Courses and International Internships during their last two semesters, but it is not recommended.

What are the GPA requirements?

  • Exchange All students must have 3.0 minimum GPA.
  • International Internship, Third Party Students must have a minimum 2.0 GPA.
  • JGE Courses Undergraduate students must have a minimum 2.0 GPA. There is not a minimum GPA requirement for graduate students.

What other requirements are there?

  • JGE Courses – There are no minimum UTD credit hours to participate in a JGE Course.
  • Exchange, Third Party, International Internship:
    • Undergrad students must have a minimum 30 UTD credit hours
    • Graduate students must have a minimum 9 UTD credit hours
    • Freshman/Sophomore transfer students must have a minimum 30 UTD credit hours
    • Junior/Senior transfer students must have a minimum 15 UTD credit hours

I’ve applied online. Now what?

I’m a fast-track student. Are there any requirements I should know about?

Just like any other student, fast-track students cannot study abroad their last two semesters of their undergraduate careers. If students are going to fast track, they should consider doing an international internship or JGE Course.

I’m about to graduate! If a student only has 2 (or less) semesters left, but they need to complete a study abroad requirement, what should they do?

  • In this case, the student should consider doing two separate JGE courses.
  • If they have time, they could also consider doing an international internship.

Are there any specific UTD scholarships available?

  • Yes! JSOM students have two ways to apply for study abroad scholarships at UTD: The International Education Fund Scholarship (IEFS) and the JSOM Travel Scholarship. However, in order to be eligible for the JSOM Travel Scholarship, the student must first have applied for the IEFS.
    • JSOM Travel Scholarship – $200 a week for Exchange/Third Party/International Internships; $500 for a JGE

What other scholarships are available?

  • McDermott Terry Scholarships fully fund international internships.
  • National Merit Scholars get $6,000 grant for international travel.
  • Students receiving a Pell Grant are eligible for $5000 Benjamin A Gilman International Scholarship.
  • The Davidson Management Honors Program has funding available for their students.

Will my scholarships such as the Academic Excellence Scholarship (AES) still apply even if I’m spending a semester abroad for an exchange program?

  • As long as the student still meeting the GPA requirements and enrolled in the required amount of hours, his/her scholarships such as AES will still apply.

I am a current student of the on-line LPN program. Although I cannot speak for the daytime program, I would advise you to stay well away from the on-line program. The course work is very poorly planned, it is almost impossible to get any sort of assistance from the instructor (she teaches this course, is a full-time nurse practitioner, is in med school, and also recently adopted a baby), and a good deal of the administration has no idea what the heck is going on.
This has been one of the worst educational experiences I have ever had. (Most of my classmates agree.)
Again, I am speaking specifically of the on-line program. I have heard mixed reviews on the daytime course. If you do go with PTEC, try to register at the St Pete campus. They seem to keep it together a bit more than the Clearwater campus.
Personally, I would rather have paid 4 times the cost and have gone to a better school.

that is very discouraging to me. especially when i’ve completed my entire aa degree online via spc, with the exception of a few classes that need to be taken on campus. i was so looking forward to being accepted to the august class, now i’m doubting i’ve made the right decision. i turned down galen after being accepted. moreso because of the expense to attend.

what specifically do you not like about the online part of the course? i know from the classes i’ve taken, that there is a ton more work to be had for the online students, as opposed to those who attend on campus, but that is to be expected when taking classes online. (at least from my experience)

reinvent. i heard there were less than 40 applications turned in for the program starting in august. that is somewhat encouraging news, considering the amount at the orientation here’s hoping we both get into the program

and for the record: i think you get what you give to an online course. there is a lot of self-discipline needed to succeed in any course, but more so in an online course. don’t let anyone discourage you — if you give 100%, you will reap the benefits in the long run.

I agree. I’ve taken online classes for the last few semesters so I’m pretty used to it by now. Not to mention that it was the first online class PTEC has tried so that is probably why it was poorly planned. The second time around they should have learned from what was good and what wasn’t and improve.

What amount of points did you have? I had 6 plus the the 8 points for the READI. Wow, I can’t believe there was less than 40 because there was a LOT of people at that meeting. Hopefully it works out for us then :spin:

well, i’m not sure — and i’m SHOCKED that you didn’t get in either! i was told that a lot of the ones accepted, had high TEAS scores, but their points were 15 and up. they were previous applicants – so there’s 1 extra point, some millitary, so there’s another extra point – and i’ll assume male as well. it just seems impossible to me to even compete to get into these programs. i don’t want to spend the money, but maybe a private school is the only way to really get in. i turned down galen (May start date) because i wanted to get into this program — to save money for one reason, but really because i couldn’t see spending all that money on a private school to get the same outcome in the end. seems to me that those private schools are only out to make money — i mean, how can EVERYONE get into them, but few get into these?

they wanted me to apply to the october day class, but it’s in st. pete and unless i could find an evening hour job, i couldn’t do that. i suppose i could just wait it out and see what happens in january, but i’ve been WAITING for so long now, i’m starting to wonder if it is worth it.

i’ve also heard that local hospitals are no longer hiring LPNs, so that leaves long term care as the only place for employment. wouldn’t mind that, but the pay isn’t there either. i really wanted to get in the pTEC program to bridge to the SPC RN program. i’m really n ot sure what i am going to do. how about you?

Are you serious? I only scored a 85.3 on my TEAS and received an email saying I was accepted as the first alternate if anyone should decide to drop from the original roster. But I also had some points from having my A.A. degree and working in the medical field. It really sucks because considering it’s an online program, I really doubt anyone would decide to not do the class unless they find out they got into a R.N. program as well.

So, what are you gonna do now? Are you gonna wait to reapply for the next class in January or try something else? I’m still not sure.

Paralegal

When it was launched, the Paralegal program at Centennial College was a direct response to the demand for paralegals, which continues today.

Because the Law Society of Upper Canada (LSUC) accredits this one-year undertaking, upon successful completion of the Paralegal program you are eligible to write the mandatory LSUC licensing examination that is required to practice as a paralegal in Ontario.

During your time in Centennial College s Paralegal courses, you will gain theoretical and practical experience, practicing and competing in mock trials. Additionally, part of the final semester is devoted to a two-day-per-week work placement that totals 196 hours more than the experiential learning requirement of the Law Society.

Once you complete the program, you will be an integral part of the legal system from the first client meeting until case resolution. You will have the distinction of being allowed to represent clients in criminal code offences, traffic violations, municipal offences, various boards, agencies, commissions, and tribunals as well as small claims court.

Semester Note

In the third semester, students will participate in a field placement (two days a week and the rest of the week is devoted to other classes) for a minimum of 196 hours. Students must have successfully completed all Semester 1 and 2 courses with an overall grade point average (GPA) of 2.0 in these courses.

Minimum Program GPA of 2.0 (C Grade average) required to graduate.

Online Occupational Therapist Degree Program Information

Essential Information

According to the U.S. Bureau of Labor Statistics (BLS), occupational therapists must earn a master’s degree from an Accreditation Council for Occupational Therapy Education accredited program. Such programs are available online, as are doctoral occupational therapy programs for those considering higher education, but on-campus programs are much more prevalent. Occupational Therapist Doctoral Degree

As of May 2015, the median wage for occupational therapists was $80,150, according to the BLS. The highest-earning ten percent of occupational therapists earned over $116,030.

Occupational therapy careers will become more in-demand due to an increase in the population of individuals with disabilities requiring therapy according to the BLS. The rise in the elderly population will also increase demand for occupational therapists (www.bls.gov ).Therapists who work with the elderly may help patients’ live independent lives by identifying safety hazards in the home or assessing their ability to perform basic functions such as driving.

Online degree programs for occupational therapists are available at the graduate level and are designed for students who are already licensed and practicing occupational therapists.
